Enrollment Agreement

Applicant / Student (hereinafter referred to as "Student") has had the course(s) fully and completely explained to him/her and agrees to abide by the terms listed in the Enrollment agreement and by the rules and regulations of International Dive Management (hereinafter called "School"). This agreement consists of one page, front and back and consists the entire agreement between the School and the student. The Student understands that if this agreement is accepted by the School it shall be deemed a binding contract.

I hereby understand that the balance of my tuition fee for the course must be paid on the starting date of the course. The appropriate PADI fees are not included in the course tuition and are due to be paid by the student at the end of each program.

I authorize investigation of all statements made in all applications and physical examinations. I understand that misrepresentation or omission of facts called for may be grounds for dismissal.

It is understood that this form in no way implies that the school guarantees to graduate or supply certificate of proficiency to all members or for that matter any member of any class. Only those earning a certificate will receive one. The respective certifications to be awarded for the course enrolled.

I also agree and understand that I may be expelled or terminated upon notice if I do not observe the schools rules and regulations or if I am found to be temperamentally unsuited for diving.

REFUND POLICY

A. An applicant may cancel his enrollment at any time before the commencement of his classes. An applicant not requesting cancellation by his scheduled starting date will be considered a student

An applicant canceling enrollment will receive a full refund of tuition and fees if International Dive Management is notified within three (3) days of signing the enrollment contract.

  1. Any applicant canceling enrollment after three (3) days of signing the enrollment contract will receive a full refund tuition minus a registration fee equaling 50 % of the required deposit not to exceed $ 100.00
  2. Any applicants rejected by Dalis Market will receive a full refund of all tuition and fees paid.
  3. All refunds will be made within thirty (30) days of cancellation or termination.

B. A student termination will be considered to have occurred on the last day of actual attendance at the school unless earlier written notice of termination is received by the school, in which case termination will be made the date of receipt of written notice.

  1. Students voluntarily terminating and those dismissed by International Dive Management will receive a refund prorated on an hourly basis.
  2. There will be no refunds issued once one third or more of the program is completed.
